Community Support Services (CSS)
Community Support Services (CSS) refers to mental health rehabilitation services and support, provided by CSS program staff. CSS staff is comprised of a licensed clinical social worker (LCSW), a registered nurse (RN), and direct support staff who are bachelor’s level staff.
In CSS consumers can receive group and individual sessions, to assist them in reaching their treatment goals. We also offer in person and telehealth appointments. Direct support staff is able to work with consumers on goals related to budgeting, nutrition, mental health wellness, and activities of daily living.
A comprehensive needs assessment is completed with each client. Clients learn skills in a person centered, non-stigmatizing environment. Skills are discussed, taught, modeled, and practiced in the community. Skills such as: Independent living/ADL skills, Social skills, skills related to accessing & using health service (e.g. asking the doctor questions), skills related to accessing & using entitlements, health and wellness promotion skills, cognitive skills (e.g. problem solving), and work readiness skills. Health, wellness, and recovery-oriented treatment is the focus in order for clients to achieve community integration and remain in an independent living setting of his/her choosing. Direct Support staff work with clients in their homes and in the community. CSS encourages community integration and actively participating in the community, while helping the client identify their valued social roles. Staff will place a heavy focus on illness management, crisis intervention, as well as coordinating and managing services.
