Release of Medical Records

Release of Medical Records:

To request a complete copy of your medical record or radiology reports and images, or to send a copy to a third party, please download and complete the Authorization Form. You may return the completed form to the Medical Records Department by fax at 973-266-4572 or by email at  roi@carewellhealth.org

We can provide copies of your medical records via secure email or we can mail them to you directly.

Requests from attorneys can be mailed to the:

We do not accept legal requests where it requires services of process (e.g., a complaint, counter-complaint, Order to Show Cause, etc.), it must be sent to the hospital’s registered agent – Corporation Service Company (CSC), it must be sent to:
